   ARE YOU A CANDIDATE FOR GASTRIC BYPASS SURGERY?

     (All four criteria must be met)

  1. A Patient must weigh a minimum of 100 pounds over their ideal body weight, or two times their ideal body weight. The ideal body weight is determined by standards of height and weight established by Metropolitan Life Insurance scales of 1982 (or later)

  2. A history of morbid obesity of more than three (3) years duration.

  3. Failure to lose weight despite documented participation in at least one formalized weight control program.

  4. There must be a medical condition directly caused by, or made worse by the extra weight, such as:
  5. Hypertension
  6. Cardiovascular disease
  7. Diabetes
  8. Sleep apnea
  9. Chronic respiratory insufficiency
  10. Dependant edema
  11. Chronic degenerative joint disease of knees, hips, ankles or back (including gouty arthritis)
  12. Pickwickian syndrome
  13. Severe gastroesophageal reflux disease
  14. Chronic depression as a result of obesity
  15. Hyperlipidemia (elevated cholesterol or triglycerids)
  16. Gallbladder disease
  17. Recurrent incisional hernias
  18. Chronic phlebitis and/or chronic venous insufficiency
